What is the purpose of an LMN?
Justification: Proves to insurance providers that a service is a medical requirement.
Coverage: Assists in securing approval for specialized treatments or therapies.

An LMN includes essential patient and provider details—such as full names, insurance IDs, and clinician signatures—alongside a formal clinical diagnosis of the medical condition. It also outlines the specific treatment plan, including any recommended therapies or equipment, and provides medical evidence like test results or clinical guidelines to justify why the care is a necessity.
Should I arrive early?
Yes, it’s recommended to arrive at least 15–30 minutes early to check in, change, and unwind before your treatment.
What do I need to bring?
Generally, you don’t need to bring anything; we provide all necessary amenities to ensure your comfort and relaxation.
Is a consultation required?
Depends on the service, an Initial Consultation and Patch Test can be done the same day. Initial consultations allows for discussion of your goals, review your medical history, and determine if you’re a good candidate. You’ll discuss color, shape, and any customization options. Patch test may be recommended to ensure you don’t have an allergic reaction to the pigments or numbing agents used.
How long does the permanent makeup last?
The results are semi-permanent to permanent, lasting anywhere from 1-5 years or longer. Fading may occur over time due to skin renewal, sun exposure, and other factors, so occasional touch-ups may be needed.
What To Expect for Follow-Up Appointments?
Permanent makeup often requires a touch-up session after 4-8 weeks to perfect the color and shape.
